The Office of the University Registrar strictly follows University Guidelines for the Honor Code. Please be aware of this when filling out any of the forms listed below.
Please fill out the appropriate form, obtain all required signatures, and return the form to the Office of the University Registrar. If any forms are due to the Office of the University Registrar on any of the dates listed on the academic calendar, they must be submitted by 5:00 p.m. on the deadline. Please have a valid photo I.D. when picking up and dropping off forms in person at the Mason Student Services Center. Electronic photos or copies of a photo I.D. will not be accepted.
All forms submitted are normally processed in two to three business days, not including the date received. There are several peak periods, (graduation, the priority registration period etc.) when processing time may take longer.

Special Registration to Retain Active Status (ZREG 200) | All Students | For students not enrolled in a credit-bearing course but whose academic department certifies that they are pursuing an activity related to their Mason enrolled program can retain active status by registering for Special Registration (ZREG 200). |
Student Success Plan | Undergraduate Students | Students requesting permission to repeat a non-repeatable course beyond the allowable limit use this form to document a plan for completing a course successfully. |
Study Elsewhere | All Students | Used to take coursework at another regionally accredited U.S. institution. Must be submitted to the Office of the University Registrar prior to taking the course by the set deadline. |
Study Elsewhere Removal | All Students | Submit to the Office of the University Registrar to remove a previously requested Study Elsewhere. On your return, the final graded transcript must be sent to Mason. |
Substitution/Waiver Form | All Students | Request that a requirement in an academic program be met by: 1) a transfer course even though not considered equivalent to a Mason course, or 2) a Mason course not usually applied to meet the requirement. Also to request that, on some clearly-detailed basis, a requirement in the student's academic program be waived. (Does not waive or give credit hours.) |
Termination Exception Request (Graduate) | Graduate Students | Submit this form to your College upon notification of possible Termination. |
Time Conflict | All Students | For students requesting to register for a course that results in a time conflict of 15 minutes or less |
