Health coaches are embedded within a care team, providing support and expertise when needed

You’ll help patients move toward healthier lifestyles, getting to know them and build trusting relationships over regular appointments and follow-up visits. You’ll help them find new ways to meet health goals and partner with them to navigate issues affecting their progress.

Help patients meet their health goals through proven behavior change methods

You’ll also lead the Whole Health Council — which gathers feedback from patients to help inform further services and education to make care more accessible and beneficial — and attend events to spread knowledge and raise awareness.

Every day I get to walk alongside patients

as they consider the possibilities, make positive changes, and celebrate their successes. To get to do all this with a team of like-minded coworkers simply adds to the satisfaction.

Health Coach Role

For us, "health coach" isn't a title that just anyone can use. Help people gain control of their health through empathetic listening and behavior change.

All Vera health coaches are certified through a stringent accreditation program and trained in empathetic listening along with the Transtheoretical Behavior Change Model by our team of coaches.

As a result, our coaches are well-equipped to work directly with patients, providing the guidance and support they need to change habits, make informed and proactive decisions about their own health, and meet lifelong goals.

As a fully integrated part of the primary care team, health coaches are able to provide a path to behavior change that works with each patient’s whole health plan.
